Vaginal Rejuvenation

Vaginal rejuvenation is a concept of making vaginal tissue younger and healthier. Various treatments are being advertised as vaginal rejuvenation. We have new modalities entering the field of vaginal rejuvenation and even in Dr Novikova’s practice new treatments have been introduced over the past few years.

The field of vaginal rejuvenation is old and new at the same time. Women knew about the tightening qualities of hot stones inserted into the vagina from ancient times. It is not the treatment that women should be trying nowadays as the results are unlikely to be consistent and the risks of harmful side effects are significant. New technologies such as laser, radiofrequency and various serums and medications are all advertised widely but have not been studied sufficiently. Good quality research takes years of planning and significant funding. The gold standard studies are randomized, controlled and double blinded trials where risk of bias is minimal. There are no randomized, controlled trials that have been conducted on the various modalities of vaginal tightening. What Dr Novikova offers to her patients is based on her patients’ needs and her expert opinion.

No, the treatment is not painful. You may feel discomfort on insertion of a vaginal probe especially if you have not been sexually active for a while. • certain medical disease (for example scleroderma, uncontrolled diabetes)
  • taking medications that thin blood
  • vaginal bleeding
  • pregnancy
  • cancer
  • pelvic organ prolapse stage more than 2
  • unrealistic expectations
  • Pap smear should be done within the last 3 months before the procedure

  • you should not have sexual intercourse for 3 days
  • you should not use tampons for 3 days
  • please phone your doctor or the office if you have bleeding, fever or any other concerns
    regarding the procedure
  • it is very common to experience increased vaginal discharge sometimes bloodstains for several days after the treatment
  • very rarely women develop involuntary leaking of urine for 1-2 days after the treatment, this is very uncommon

You can expect to feel the full (maximum) results 3 months after your last 3rd procedure. Some women (especially these suffering from vaginal dryness and recurrent infection) feel a significant improvement of their symptoms after the 1st treatment. Women with stress urinary incontinence may feel some improvement after their 2nd session, but the full effect is felt three months after the last treatment.

Generally, 3 treatments are enough to achieve the desired result. If the result has not yet achieved you may need an alternative treatment such as the sling procedure for stress urinary incontinence or a vaginoplasty for vaginal tightening. Occasionally doctors will choose to do more treatments (usually 4-6 treatments every 2 weeks) if lower energy of laser has to be used during treatments.

Depending on the problem, vaginal laser treatment improves symptoms of atrophic vaginitis in the majority of women, it works for mild to moderate stress incontinence in 4 out of 5 women. We do not know how well vaginal laser treatment works for vaginal tightening.

No, vaginal laser treatment does not leave scars.

First, what is the actual problem and severity of it:

  • Aesthetic issues only such not liking the look, but satisfied otherwise in terms of sexual function
  • Vagina laxity or looseness with little friction during sexual intercourse where vaginal penetration results in a lack of sensation
  • Vaginal dryness and painful vaginal penetration
  • Combination of the above

Once we know what the issues are we can consider various treatment options.

  • Aesthetic concerns will be answered with either surgery or a particular treatment to address the concern
  • Vaginal laxity or looseness can be addressed with surgery (if it’s severe). The surgery is called vaginoplasty or vaginal repair and is performed in an operating room under general anesthesia. It requires 2 weeks of down time and avoidance of sexual intercourse as well as strenuous exercise for 6 weeks.

Vaginal laser treatment can be performed with various lasers (CO2, Yag/Erb, hybrid – Femilift, MonaLisa, Gynaelase) and has similar effect to vaginal radiofrequency treatment (UltraFemme Exilis 360, Vivive, Thermiva).

Although the research using the above-mentioned devices in vaginal rejuvenation is still limited, the device has been used on hundreds and thousands of women with good results and minimal risks.

When choosing the device it’s important to take into consideration whether the device is well known, registered with the FDA and whether the person performing the treatment is well qualified, understands pathology, can diagnose other possible conditions resulting in abnormal symptoms and can deal with failures or complications of treatment.

In her practice Dr Novikova uses two devices for vaginal treatments – CO2 fractional laser Femilift and a radiofrequency device called UltraFemme. Both devices achieve the vaginal tightening effect through increasing temperature in vaginal skin in a very controlled manner, shrinking of collagen fibers, stimulation of production of new collagen in the tissue. Both devices have a positive effect on vaginal dryness or atrophy, urinary incontinence and frequent vaginal infections by improving the blood flow to the whole vaginal area, increasing lubrication, boosting immunity and building strength and elasticity.

The Femilift producer has been on the market for decades and the company is a leader in laser industry. It’s the only vaginal laser that has disposable probe to cover the vaginal hand piece which avoids the transmission of any infections.

Ultrafemme is the only device on the market that delivers consistent temperature increase at 360 degrees when the treatment is administered. Radiofrequency penetrates tissue deeper than fractional CO2 laser. There is no down time after radiofrequency treatment, but no sexual intercourse is allowed for 3 days after laser treatment.

Other types of treatment:

  • V-sculpt is a device for home use. It is a good option for women who prefer to do treatments themselves at their own time and without affecting their privacy. You can purchase the device for R9000. The treatment plan is a session every other day for 45 days.
  • Emsella Chair is a pelvic floor exerciser and a non-invasive treatment that addresses pelvic floor muscle strength. A woman undergoing this treatment experiences 11000 pelvic muscle contractions in a 28 minute session.
  • AQ vaginal serum delivers growth factors to the vaginal skin stimulating production of collagen and assisting with vaginal dryness.
  • PRP-genital treatment or O-shot delivers the patient’s own growth factors extracted from their blood which also stimulates production of collagen and rejuvenates vaginal skin.

Combination treatment are possible, for example vaginal radiofrequency or laser treatment will improve vaginal skin. Emsella pelvic floor muscle treatment will strengthen pelvic floor muscles. Therefore, combining two of these treatments will address different areas such as skin and muscle and potentially accomplish a better result.

The choice of treatment would depend on the problem, severity of the problem, life-style circumstances and feasibility of treatment for the particular patient.
